Artificial intelligence has now well and truly shifted from being an emerging experiment, to an operational reality in healthcare.
Machine learning models now assist radiologists in image interpretation, support emergency triage, predict patient deterioration, and even shape workforce planning. We know this transformation carries immense promise – improved outcomes, reduced administration, and data-driven decision-making.
Yet, as with every great opportunity there also comes great risk – opaque algorithms, ethical dilemmas, safety concerns, and blurred lines of accountability.
As the Australian healthcare system accelerates its digital maturity, clinical and digital governance structures must also evolve, preferably in tandem.

Effective AI clinical governance begins with alignment. More specifically, with ensuring that digital investments are serving clearly defined clinical purposes.
Too often, AI initiatives are driven by technical enthusiasm or vendor opportunity rather than clinical need. The result of this then is a disconnect between what a model does and what clinicians actually required at the bedside in the first place.
This misalignment can manifest in 3 main ways:
- decision-support tools that disrupt workflows instead of supporting them,
- algorithmic outputs that lack transparency, and
- accountability gaps when technology-influenced decisions lead to harm.
Healthcare organisations must therefore design governance frameworks that unite clinical, digital, and operational domains.
AI implementation should be guided by clinicians who understand patient care, supported by data scientists who understand model performance, and governed by executives who understand system-wide risk.
Principles for 21st-century clinical and digital governance
Governance in the AI era is not about more bureaucracy. It needs to be about better clarity, shared values, and adaptive oversight. A set of foundational principles that can serve as a compass in this regard, includes the following:
- Clinical primacy must remain non-negotiable. Every AI tool should be evaluated primarily by its impact on patient outcomes, safety, and clinical appropriateness.
- Transparency and explainability are equally important. Clinicians cannot safely use tools they do not understand, and patients will not trust systems that cannot be explained.
- Governance must ensure continuous validation, as models can drift over time when clinical practices, population demographics, or data inputs change.
- Equity must be embedded from the outset, by assessing and mitigating bias to prevent AI from inadvertently widening disparities.
- Accountability and traceability are core governance principles that must be upheld, so that each AI-driven decision can be audited for who made it, on what basis, and with what safeguards.
Obviously these principles should be applied with proportionate oversight and common sense, by matching the level of governance to the degree of clinical and operational risk. As an example, an algorithm that allocates administrative shifts would not need the same scrutiny as one that assists in diagnosis or treatment planning.
Clinical governance
Clinicians at the center
Clinical governance is what provides the ethical and operational foundation for safe AI use. Clinicians must be engaged at every stage of the AI lifecycle: from defining the clinical problem and curating datasets, to setting performance thresholds and monitoring real-world outcomes. Their insights are what can ensure that technology actually responds to clinical need, rather than just pointlessly dictating it.
AI use-cases must also be clearly defined. Every tool should have an identified purpose, specified users, and clearly documented boundaries for decision-making. A diagnostic support model, for example, must be explicit about whether it provides advice, prioritises images for review, or makes autonomous interpretations.
Additionally, AI clinical governance frameworks should define robust escalation and override protocols. Clinicians need a transparent process for raising concerns or overriding AI outputs for when and if their judgment differs.
Embedding AI performance into existing quality structures in hospitals, such as incident reporting systems, morbidity and mortality reviews, and credentialing processes, can create that vital continuity and seamless integration between traditional clinical governance and the new digital frontier that is required.
Digital governance
Data and operational integrity
If clinical governance is what safeguards clinical care, then digital governance is what safeguards the infrastructure and processes that enable it. This is why AI clinical governance must span the full lifecycle of models, from conception to decommissioning, ensuring that data, algorithms, and integrations remain reliable over time.
Strong data governance underpins everything. Health leaders must invest in the basics. These include robust de-identification processes, documentation of consent conditions, and metadata catalogues that can trace where data originated and how it has been used. Otherwise, poor data governance will translate directly into poor model performance and ethical vulnerability.
Models should be versioned, validated, and retired in a controlled way, much like medications. This means implementing performance monitoring, drift detection, and revalidation processes to ensure models continue to operate safely and as expected in changing environments.
Procurement and vendor management are additional aspects that require appropriate governance considerations. Executives should require full transparency from AI suppliers, including access to model documentation, validation data, and ongoing update protocols. Contracts should allocate accountability clearly, including who is responsible for adverse outcomes, software maintenance, and retraining.
Finally, interoperability and integration matter deeply. Good governance must ensure that AI systems work harmoniously with existing electronic medical records, workflows, and clinical decision systems. Poor integration can increase cognitive load, fragment care, and even introduce safety hazards, which is the very opposite of what AI promises to achieve.
Risk, regulation and ethics
Building a defensible posture
Regulatory frameworks are rapidly evolving, and health leaders can no longer afford to wait for clarity before acting. Adopting a proactive governance posture would mean mapping regulatory obligations across privacy, data protection, and medical device laws. It also means preparing to comply with international standards for AI management systems by aligning internal policies.
Ethics, too, must be operationalised and not confined to committees or mere statements of intent. Every AI project should undergo a rapid, pragmatic ethics review that assesses fairness, transparency, and patient autonomy. These reviews should then feed back into project design rather than sitting as retrospective approvals.
Finally, AI clinical governance frameworks should include defined incident response protocols for AI-related errors or harm. These plans should set out how to investigate model performance, protect patient data, communicate transparently, and learn systemically from incidents, without discouraging innovation.
Workforce and capability
Investing in people, not just platforms
AI adoption in healthcare will only be as strong as the capability of the workforce that uses it. This is why investment in education and multidisciplinary collaboration is a cornerstone of good AI clinical governance. Clinicians, executives, and managers must all understand the fundamentals of AI such as what it can do, where it fails, and how to interpret its outputs safely.
Organisations should foster teams that blend clinical expertise with data science, legal, and privacy knowledge. These multidisciplinary groups bring the balance required to evaluate AI from multiple lenses: clinical efficacy, ethical safety, and operational feasibility. New roles such as clinical informaticians, model operations specialists, and data stewards are also now emerging. These need to be formally recognised and supported through workforce planning.
Measurement
Defining what “good” looks like
Robust AI clinical governance depends on measurement. Organisations should develop dashboards that monitor a blend of clinical, operational, and algorithmic performance indicators. These may include:
- patient outcomes (such as diagnostic accuracy and adverse event rates),
- workflow efficiency (such as alert fatigue or turnaround time), and
- model performance metrics (such as false positive/negative rates and calibration).
Critically, results should be monitored for equity among population groups. For example, performance across gender, ethnicity and socioeconomic status, ensures that AI enhances fairness rather than undermines it.
Transparent reporting of these to governance committees and clinical leaders will then help to sustain accountability and trust.
Implementation road-map
Translating principles into action
Strengthening AI clinical governance need not be overwhelming. Healthcare leaders can begin with simple, pragmatic steps:
- Start by cataloguing all AI tools currently in use or under procurement, classifying them by clinical function and risk level.
- Use this inventory to prioritise high-impact areas for oversight.
- Design or update governance structures to explicitly include digital health, data science, and consumer representation.
- Require pilot projects to meet safety and evaluation criteria before scaling, and implement ongoing monitoring systems that can inform real-world model performance.
- Ensure that every governance process is responsive and agile enough to adapt as AI capabilities and regulatory expectations evolve.
The leadership mindset
Humility over hype
AI is not going to replace clinicians, but it is going to amplify their capability. But the danger with this is that amplification without adequate oversight can lead to distorted judgment and harm to patients. Healthcare leaders must therefore cultivate a mindset of “humility over hype” and yes, embrace innovation, but while still maintaining a disciplined, evidence-based approach to its adoption.
Good AI clinical governance is what provides the scaffolding for getting this balance right. It can create the confidence to explore AI’s potential without compromising safety or ethics. This can then shift AI from being a technological risk to a clinical and strategic asset.
Turning governance into a competitive advantage
AI is rapidly redrawing the boundaries of clinical practice, patient engagement, and operational management. In this type of landscape, clinical governance should not be a barrier to innovation, but rather an enabler.
The health systems that invest now in robust, clinically integrated digital governance will be the ones that move fastest and safest in realising AI’s promise.
