Drawing on the Australian policy context and frontline operational insight, we outline a practical framework for aligning ED funding with true clinical complexity.

The paper examines the limitations of existing models, defines measurable markers of complexity, and proposes funding adjustments that better reflect workforce intensity, coordination demands, and risk profile. For health service executives, policy leaders and clinicians, this paper offers a roadmap to move beyond throughput metrics, toward funding that supports safe, high-quality care for the patients who need it most.

Pay-for-Performance in Australian Healthcare

AIHE supports a fair, evidence-based Pay-for-Performance approach that rewards outcomes improving patient safety, quality and value. Traditional funding models have focused on activity rather than results. We advocate for incentives that align financial sustainability with better care.

Effective Pay-for-Performance must be grounded in strong clinical governance, equity, and transparency, encouraging improvement rather than punishment. AIHE calls for clinician-led design, robust risk adjustment, public reporting, and reinvestment of savings into workforce and quality initiatives.

This position statement outlines the shared position of AIHE and Board Benchmarking: that the CLEAR framework represents a significant step forward in care services governance, offering leaders a tested and adaptable framework to strengthen oversight, ensure quality and safety, and embed consumer voice at the heart of care.

The CLEAR Framework: Strengthening Clinical Governance for Health, Aged and Disability Care Boards is a jointly developed new framework to assess the clinical governance maturity of healthcare, aged care & disability care boards.
